Overview
Members can personalize their experience to see the value and benefits of Wellframe from the first day they download the application. Newly onboarded and existing members can receive an overview of the app, submit a self-reported health goal to their care team, request to enroll in a program, set up a healthy habit or medication reminder, and start a conversation with their care team.

Staff can view the member’s self-reported goal (and additional conditions when applicable) on the dashboard within the Member Profile View.
During the activation flow, members will see the following screens:
- Welcome & Application Overview: introduces member to the activation flow and describes the application features
- Care Team Overview: describes what a care team is in the context of Wellframe
- Health Goal Selection: introduces choosing a health goal and requesting enrollment into an applicable care program
- Health and Medication Reminders: allows members to set up reminders and notifications
- Care Team Introduction: prompt to say Hi to the care team with a pre-populated message
Care Manager Experience
Staff will see the member’s selected health goal in the dashboard within the member profile.
Health Goal Selection
During the health goal selection flow, the member is asked “What is the main health goal you would like support with?” and can choose from a list of goals based on the care programs available for that member.
Based on their health goal selection, members may see additional questions. For example, the “Lose weight” health goal includes an additional question “Would you like to lose weight to manage a condition?” and then lists a series of conditions the member can choose from.
Once entered, the health goal will display in their profile. This currently cannot be edited.

Care Program Request to Enroll
After selecting a goal, members will be presented with optional care programs associated with their selected goal. Members may request to enroll in a program of interest to them continue without a care program request.
Health and Medication Reminders
Members can add health and/or medication reminders and are prompted to enable push notifications on their mobile device.
Resuming Personalization
If a member exits the activation flow at any point, they can resume it from their Checklist using the Jump In button.
