Most members are put into the Welcome Program as a default when they onboard. This program does NOT focus on their specific health goals or conditions. We recommend putting them into on of the below condition specific programs to better support their journey.

Prediabetes Support A 35-day program that provides education and support for people with prediabetes, and emphasizes the opportunity to prevent the development of type 2 diabetes through therapeutic lifestyle changes, and formation of better habits. Members will gain information and support about symptoms, therapeutic lifestyle changes, and important screenings and lab tests. This care program will help you develop better habits and a healthier lifestyle in order to avoid developing type 2 diabetes. Designed for people with a prediabetes diagnosis or a concern about a future diagnosis, you’ll learn about symptoms, therapeutic lifestyle changes, screenings, and lab tests. Prediabetes is reversible, so information is power to make changes that matter for your long-term health. Chronic Disease: Hyperlipidemia A 30-day program that delivers education on hyperlipidemia causes and risk factors, pathophysiology, treatments and medications, and follow-up. Lifestyle changes and emotional health are also reviewed at length. Hyperlipidemia may not have symptoms, so it can be hard to understand why managing it matters. Hyperlipidemia can lead to complications like heart disease, so taking the steps to stay on top of it can reduce your risk of future problems. There are a lot of changes you can make to keep yourself healthy. This care program provides educational content to teach you more about the causes, risk factors, treatments, and lifestyle changes for hyperlipidemia. You may not feel symptoms of high blood pressure on a daily basis, which may make it harder to see the damage it can do. Care Transitions: Cardiac Rehab A 3 month companion program for people engaged in an outpatient cardiac rehabilitation program, that provides a 12-week, structured curriculum. Weekly modules cover heart structure and function, cardiac problems, heart healthy eating, physical activity, emotional health, goal setting, changing habits, managing healthcare, and maintaining change. This program should be layered with a Care Transitions foundation program. It should not be used as a standalone program. While you’re in cardiac rehab (and after), you’ll need to keep an eye on your heart health. This care program helps you manage your health while in cardiac rehab. You’ll learn more about topics like the anatomy of the heart, common heart complications, goal setting, and how physical activity, emotional well-being, and diet influence your cardiovascular health. Content covers AV fistulas, AV grafts, and temporary vascular access. Before you start hemodialysis, you’ll need vascular access. This care program will teach you how to prepare for vascular access and what the common types are. Your care team will be there to support you and answer any questions you have. This program provides multiples-specific educational content on topics including defining a multiple pregnancy, nutrition, emotional health, and parenting. This program should be used in coordination with the general pregnancy educational content that is delivered in the Maternal Health: Prenatal & Postpartum Support program. When you’re pregnant with twins, triplets, or more, there is more to learn than having just one baby. This care program covers what you need to know about having multiples, from nutrition to parenting to emotional health. Getting support during your pregnancy journey can be helpful, whether this is your first pregnancy or you’ve been a parent for a while now. This care program provides educational content and prenatal support during the entirety of your pregnancy, in addition to 6 weeks of postpartum education. This program provides educational content on the effects of gestational hypertension, monitoring blood pressure at home, a heart healthy diet, and safe exercise. This program should be used in coordination with the general pregnancy educational content that is delivered in the Maternal Health: Prenatal & Postpartum Support program. Keeping your blood pressure under control is important for you and your baby’s health during and after pregnancy. This care program provides educational content to help you manage gestational hypertension. Learn about the effects of gestational hypertension, how to monitor blood pressure at home, what makes for a healthy diet, and safe ways to exercise. Maternal Health: Gestational Diabetes A 10 day supplemental program for women that have been diagnosed with gestational diabetes. Note: This program includes a companion downloadable toolkit for members that provides support in navigating perimenopausal care options with providers. This content is also provided in nondownloadable format in the program. The transition to menopause and beyond is a natural part of life. Not everyone experiences this transition in the same way. For some, the transition can be a little bumpy. And for others, it can be more like a rollercoaster. No matter your experience, there are ways to make it smoother.
